Combustion Engineering, Inc.
Docket No. 99900002/81-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on April 20-24, 1981, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions, procedu.es, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or draw-ings.
Instructions, procedures, or drawiags shall include appropriate, quantitative or qualitative acceptance criteria for determining that important tivities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
The Combustion Engineering Quality Assurance Manual, dated March 1, 1977, Section 2.5.'.3, requires the return of all previous issues of a document when it is withdrawn from the Central Document Control.
Contrary to the above, two obsolete copies of the same procedure, Mfg. 08-32 on repair welding, revisions zero, and revision three were found on the shop floor.
The current revision was six.

B.
The Combustion Engineering Quality Assurance Manual, dated March 1, 1977, Section 14, paragraph 14.6.3.2, states in part:
"The specific findings and recommendations, corrective action required for the significant conditions adverse to quality as determined during the audit and/or review meeting, will be included in the report."
The QA Manual further requires that if a " finding", as opposed to a
" recommendation", is identified, then an investigation of the cause of the deficiency must be made, and the deficient areas reaudited.

2 Contrary to the above, problems identified in three audits performed in 1981, were not identified as either " findings" or " recommendations."
C.
The Combustion Engineering Quality Assurance Manual, dated March 1, 1977, Section 13, paragraph 13.6.8.1, states in part:
'"The DN's are summarized and reviewed on a weekly basis to determine if corrective action it required."
Contrary to the above, the weekly summarizations reviewed did not include all nonconformances identified on Deviation / Disposition Notices (DNs) reported for that weekly period.
This would limit the evaluation of con-ditions in regard to corrective act'on. When a part had received multiple nonconformances, only one nonconformance was-included in the summary.
For example, during the week ending April 11, 1975, five DN reports on Outer Guide Tube Assemblies, were summarized for seven nonconforming tubes with ten nonconforming characteristics.
The report did not reflect all of the ter, nonconformances, but only seven.
