In response to your letter of June 17, 1981, and in accordance with 10CFR 2.201, the attached replies address the Notices of Violation which were included as Appendices A and B witl' the referenced Inspection Report.
In addition to the need for corrective action regarding specific items of noncompliance, your letter expressed concern about the implementation of our management control systems that permitted them to occur. We have recently established a Nuclear Division with the goal of improving manage-i ment control. The reorganization has provided additional lines of super-vision which will permit closer management control and awareness of each departments' programatic requirements.
Since the Reply to Notice of Violation, Appendix B, contains information subject to Section 2.790 (d), Title 10, Code of Federal Regulations and is, therefore, exempt from disclosure, it is my understanding that this portion of this 81-04 response will not be placed in the Public Document Room and will receive limited distribution.

Description of Violation (81-04-03)
Technical Specification 6.8, Procedures, states, in part, " Written procedures shall be.
. maintained covering the activities referenced below:
a.
The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, November 1972.
." Appendix "A" of Regulatory Guide 1.33, November 1972, recommends the following procedures: "Section C.
Procedures for Startup Operation and Shutdown of Safety Related PWR
. C.22 Process Radiation Monitoring System.
." and "Section E.
Systems.
Procedures for Correcting Abnormal, Offnormal or Alarm Conditions.
E.27 Abnormal Releases of Radioactivity.
Contrary to the above, on February 4, 1981, procedures for operation of atmospheric steam relief valve effluent radiation monitoring instrumentation were not properly maintained in that Radcon Emergency Operating Procedure (REOP) No. 1.2, Emergency Use of Atmospheric Steam Relief Effluent Monitoring System, Revision 1, posted at the equipment location did not include Revision 2 to Figure 5.1.2.3, Calibration Curves-Detector Radiation Level vs. Recorder Indication, Sheets 7-10.
The procedure cover sheet carried a validation stamp authorizing its use from September 19, 1980, through March 15, 1980, although the above pages had been superceded by a subsequent revision.
Corrective Action Taken The Radcon Supervisor initiated immediate corrective action to post current revision copies of the curves at the equipment location.
Action Taken To Pr event Recurrence A controlled copy of the subject procedure has been assigned for use at the equipmenc location.
Controlled copies of procedures require appropriate signatures to verify that procedure revisions have been received and incorporated.
Date On Which Full Compliance Will Be Achieved Full compliance was achieved as of February 15, 1981.
