NOTICE OF' VIOLATION Washington Public Pmr Supply System 3000 George Washington Way P. O. Box 968 Richland, Washington 99352 License No. 46-17964-01 I
As a result of the inspection conducted on July 8,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A.
10 CFR 20.203 (f) (1) requires that except as provided in subparagraph (3) of this paragraph, each container of radioactive material shall bear a durable, clearly visible label identifying the radioactive contents.
Also,10 CFR 20.203 (f)(2) states that a label required pursuant to subparagraph (1) of this paragraph shall bear the radiation caution symbol and the words " Caution, Radioactive Material" or " Danger,
.i Radioactive Haterial." It shall also provide sufficient information to permit individuals handling or using the containers, or working.in the vicinity thereof, to take precautions to avoid or minimize exposures.
Contrary to the above requirements, at the time of the inspection, the Eberline Model 1000B calibrator containing approximately 167 curies of Cesium-137 located in the 421 Laboratory was not labeled.
This is a Severity Level V Violation (Supplement VII) 8.
10 CFR 19.11 (a) requires that each licensee shall post current copies of the following documents:
(1) The regulations in this part and in
?
Part 20 of this chapter; (2) the license, license conditions, or documents i
incorporated into a license by reference, and amendments thereto; (3) the operating procedures applicable to licensed activities. 10 CFR 19.11 (b) states that if posting of a document specified in paragraph (a)(1),
i (2) or (3) of this section is not practicable, the licensee may post a notice which describes the document and states where it may be examined.

above documents were neithar posted in Warehouse No. 4 where licensed material is stored, nor was there a nctice which describes the documents and states where they may be examined.
This is a Severity Level V Violation (Supplement VII) i C.
10 CFR 19.11 (c) requires that Form NRC-3 " Notice to Employees," shall be posted by each licensee wherever individuals work in or frequent any portion of a restricted area.
Contrary to the above requirement, at the time of the inspection, a Form NRC-3 was not posted in Warehouse No. 4, a restricted area, where licensed j

License Condition 18 states, except as specifically provided otherwise by this license, the licensee shall possess and use licensed material described in accordance with statements, representations, and procedures contained in appiscation dated January 11, 1978 and letter with enclostres dated June 28, 1978.
Item 14 b.3 of Attachment 1, to the letter dated June 28, 1978, states that perioolc inventory of radioactive sources will be performed." " Periodic inventory" is defined to be a quarterly inventory by the licensee's Health Physics Policy and Procedure Manual 11.2.14.4 issued on July 15, 1979.
Contrary to the above requirement, at the time of the inspection, only one quarterly inventory had ?,een completed on February 2,1981. Quarterly inventories also had not been performed during the period of July 15, 1979 through February 2,1981.

Pursuant to the provision of 10 CFR 2.201, Washington Public Power Supply System is herehy required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause show.
The responses directed to this Notice are not subject to the clearance procedures of the Office of Ibnagement and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
