This refers to the routine safety inspection conducted by Dr. L. W. Shatterly of this o#fice on July 14, 1981, of activities authorized by NRC Byproduct Material nse No. 12-17047-01 and to the discussion of our findings with Sharon Ft
' king, R.N., and selected members of your medical staff at the conclusion of the inspection, and a subsequent telephone conversation with Donald C. Burr, M.D. on July 15, 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the ccaditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, indeper. dent measurements, and interviews with personnel.
During this inspectica, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
We will gladly discuss any questions you have concerning this inspection.
