This refers to the routine safety inspection conducted by Mr. W. P. Reichhold of this office on June 25, 1981, of activities authorized by NRC Byproduct flaterial License No. 12-14998-01 and to the telephone conversation concerning our findings with you on July 7, 1981.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.

The inspection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

Our inspector expressed concern over your method of surveying generators before disposal. Appendix J of the Medical Licensing Guide 10.8 dated October 1980 states that before dispos.il, the generator columns should b7 surveyed with all shielding removed to determine if the radiation levels have reached backround. Your Nuclear Medicine Department failed to remove the lead shielding from the generator column before surveying the column.

Consequently, in your reply to this letter please confirm that you will remove the lead shielding from the generator column before it is surveyed to determine if the radiation levels have reached backround.

Our inspector also examined the corrective actions described in the letter dated February 2, 1979, signed by Gerald E. Almond regarding the apparent items of noncompliance found during our November 8, 1979, in-spection. Your corrective actions were to include the following:

(A) Daily constancy checks of tbe dose calibrator were to be performed on the commonly used radionuclide settings.

(B) Weekly G-M surveys and wipe tests were to be performed of storage areas.

(C) The method used to analyze wipe tests was to be capable of detecting 100 dpm.

Contrary to these corrective actions you failed to perform the following:

(A) Daily constancy checks of the iodine-131 setting on the dose calibrator were not performed on t;elve occasions from March 9, 1981, to June 22, 1981.

(B) Weekly G-M surveys and wipe tests of the generator storage area were not performed.

(C) Your wipe tests were analyzed using a portable G-M survey meter a method which is incapable of detecting 100 dpm.

In your reply to this letter describe your actions to ensure that items A through C will be corrected.

We have no further questions regarding Items 1, 2, and 4 of the November 8, 1978 inspection.

Item 1 of the enclosed Appendix A, is a repeat item of noncompliance.

During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required.

We will gladly discuss any questions you have concerning this inspection.
