This confirms plans discussed between you and my staff to meet with your Dr. T. Colline and other members of your staff on August 11, 1981 at 1:00 p.m. (CDT) at the Region III office in Glen Ellyn, Illinois. The purpose of this meeting is to discuss the excessiva number of reportable events that have occurred this year, the corrective actions that were taken following these events and your management plans to prevent events of this type.

We will gladly discuss any questions you have concerning this matter.

' REGION III MANUAL REGIONAL PROCEDURE 9924 PLACEMENT OF DOCUMENTS IN THE DOCUMENT CONTROL SYSTEM (DCS)

Scope l

The Document Management Branch, (DMB), Division of Technical Infor-mation and Document Control (TIDC) has established the Document Control System (DCS) to provide centralized control of NRC documents. This contrcl includes the placement of documents into the document storage and retrieval system (formerly TERA); the reproduction and distribution of designated documents within NRC; and the reproduction and distri-bution of designated documents to the local pdr, pdr and NSIC.

These procedures provide instructions for the placement of Region III documents in the DCS.

I Objective The purpose of this procedure is to assure that:

ALL appropriate Region III documents are entered into the DCS, and 2.

ALL Region III documents to be processed through DCS are submitted in accordance with procedures established by the Document Manage-

Procedures L

A.

Ceneral The Administrative Support Branch has overall responsibility for developing the procedures and guidelines for the placement of Region III documents in the DCS.

Region III will submit only originals of incoming documents and original inspection /investigati'on reports, and original signed concurrence copies of outgoing documents to DMB for processing in DCS.

(An " original signed concurrence copy" is one which bears the original concurrences and is handwritten to conform with the signed copy in the signature space.)

Submission of only originals will assure the best possible copy is available in DMB for reproduction and for microfilming and will eliminate the possibility of duplicating entries in DCS.

Should an " original" as described above not be available a copy may be processed as the " designated original" and placed in DCS. A

3-As a rule the documents submitted to DCS will not be returned; there-fore, a copy of each document submitted must be made and retained for the Region III files.

If the document is to be placed in the PDR, LPDR and /or NSIC, the document must be prepared showing this distributionfin the cc's.

/M0I 4 The original signed concurrence copy of the transmittal letter signed (by inspector) and one legible / copy of the NRC 591 will be sent to DMB on a Form 20B. The Form 20B will be prepared showing Code 4E.

No copies will be sent to IE:HQ.

(2) NRC 591 - Citing Violations In addition to the signature of the inspector the signature I

of the licensee or his representative is required on the NRC 591's citing violations; therefore, NRC 591's in.hese cases cannot be sent to DMB until the licensee has returned the signed form.

When the form has been rectived from the licensee, the original signed concurrence copy of the trasmittal letter and one legible signed copy of the NRC 591 will be sent to DMB on a Form 20B. The Form 20B will be prepared showing Code 4E.

No copies will be sent to IE.

State Liaison Officer Notification to Public Officials The original signed concurrence with notation "DCS" in upper right corner should be sent to DMB.

Piemos to HQ Re Specific Licensees Memos regarding specific Licensees should be prepared" showing the docket or license number in the subject line. The original signed concurrence with license or docket number noted in upper right corner should be sent to DMB.

Non-Docketed Documents Non-docketed documents may be placed in DCS.

Firm criteria for the identification of documents appropriate for placement in DC3 have not been developed. However, generally only documents of

" permanent, long lasting value" should be placed in the system; drafts, working papers and other documents of temporary value should not be placed in DCS.

Enclosures / Attachments to Region III Documents Enclosures or attachments that are " stand alone" documents, that is, documents which have an addressee, author and dste and are themselves identifiable, must be chetked against the DCS terminal (TERA) to determine if the document is already on DCS.

If the document is on DCS, the DCS Ac.cessio; s Number (ANO) must be written on the enclosure / attachment before it is sent to DCS. If the enclosure / attachment is not on DCS, the original copy should be sent (Note: if the original is in hand it is unlikely the document

If the original is not available a " designated eriginal" must be sent.

NUREG's, Reg Guides and NRC formal publications should not be sent to DCS separately or as enclosures.

10. _ General Correspondence (Non-Docketed Administrative Correspondence)

This includes temporary delegations, personnel related material, drafts, and working papers - not placed in DCS.

Semiannual Reports

  • These reports frequently contain data which are subject to restrictions of the Privacy Act.

However, the original incoming unsanitized report should be forwarded to DMB for entering into the system intact. After entering the report into the s/ stem DM3 will send a copy of the report to IE:HQ. An IE:HQ designee (Ralph Meyer) will sanitize the report and send a copy of the sanitized report to DME and the region.
