Enclosed is a final report of a reportable occurrence observed during a check of licenses and requalification status of NSCR operators. This report is submitted in compliance with Section 6.4 and 1.8.c of change No. 11 of the Technical Specifications, Facility License No. R-83 for the Nuclear Science Center, Texas A&M University.

i Final Report of Violation Due to Expired Senior Reactor Operator License Reportable Occurrence At 1030 on 7 May 1981 during a check of reactor operator licenses and requalifi-cation status it was discovered that License No. SOP-3478, held by Gary W.

Waldrep, had expired on 27 April 1981. This placed' the. Nuclear Science Center in violation of 10CFR55.3(b)'and NSCR Technical Specifications Section 6.la which requires that a licensed SRO be in direct control'of the facility. The Director was notified of the situation and he directed that Mr. Waldrep be relieved of his licensed duties until a new license is issued.

The NRC Operator Licensing Branch was also actified and Mr. Waldrep has submitted a license application with a re

s t to Jaive the SRO examination.

Mr. Waldrep had successfully completed the requirements of the operator requalification program at the time his license expired.

The incident was caused by a failure to follow administrative procedures.

The-individual that had the responsibility of reviewing the records of the requalifi-cation program had beea reassigned within the NSC organization.

The Manager of Reactor Operations was directed to assign the responsibility to another individual. For some reason this was not done. Unfortunately, the usual notification of license expiration from the Nuclear Regulatory Commission was not received until May 8, 1981.

Corrective Action The immediate solution to the problem has been accomplished by assigning an-operator with the responsibility of cheching the folders on all licensed operators monthly to determine license and requalification status. He will report any discrepancies or approaching license expirations to the Nuclear Science Center management.

This is the same administrative program that has been successful in the past. The recent~ failure-is attributed to personnel inadequacies and not administrative procedures. These will be corrected in the near future.
