This refers to the inspection conducted by Messrs. W. J. Slawinski and W. P.
Reichhold of our staff on March 16 and 17, 1981, of activities at your radio-graphy facility located at 5307 W. 86th Street, Ir.dianapolis, Indiana, auth-orized by NRC Byproduct Material License No. 12-00622-07.
The results of this inspection were discussed during an enforcement conference on April 21, 1981, in the Region III office between Messrs. Earl Banfield, Kenneth Bronder and Hugh Doran of Magnaflux Corporation and Mr. A. B. Davis and others of the NRC staff.
The inspection was conducted in response to a report from Mr. Earl Banfield, Corporate Radiation Safety Officer, that a radiographer had received a whole body exposure of 7.01 rems during the period February 1-15, 1981.
The inspection identified four apparent violations which are described in Appendix A.
rhe violatio..s set forth in Appendix A indicate a serious lack of control over licenseu activities.
We are especially concerned that an individual was conducting radiographic operations without performing surveys to assure that the source was being returned to its shielded position.
Consequently, we propose to impose civil l
penalties in the cumulative amount of Eight Thousand Dollars for the violations set forth in Appendix A to this letter.
The violations set forth in Appendix A have been categorized at the severity levels described in the Interim Enforcement Policy published in the Federal Register 45 FR 66754 (October 7, 1980).
You are required to respond to Appendix A and in preparing your response you should l
follow the instructions specified in Appendix A.
i Your reply to Appendix A and the findings of future inspections will be considered in determining whether any further enforcement action is appropriate.
