Anoendix A NOTICE OF VIOLATIO'i Department of the Interior Bureau of Land Management 2800 Cottage Way Sacramento, California 95825 License No. 04-15103-01 As a result of the inspection conducted on May 4,1981, and in accordance with the Interin Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A.
10 CFR 20.401(a) requires that each licensee shall maintain records showing the radiation exposures of all individuals for whon personnel V
monitoring is required.
10 CFR 20.401(c)(1) requires that records of individual exposure to radiation and to radioactive material which must be maintained pursuant to 10 CFR 20.401(a) shall be preserved j
until the Corxaission authorizes disposition.
4 Contrary to the above requirement, at the time of the inspection, personnel monitoring records were not maintained.
This is a Severity Level VI Violation.

B., 10 CFR 20.407 requires that all licensees other than those described in 10 CFR 20.408 shall within the first quarter of calendar years 1979 and 1980, submit to the Comission, the pert on.'el monitoring reports
. specified in 10 CFR 20.407(a) and (b) covering the calendar years of 1978 and 1979.
Contrary to this requirement, at the time of the inspection, the personnel ro monitoring reports for the calendar years 1978 and 1979 had not been submitted to the Comission.
This is a Severity Level VI Violation.

C.
License Condition 13. A(1) requires that each sealed source containing licensed material, other than hydrogen-3, with a half-life greater than thirty days and in any form other than gas shall be tested for leakage and/or contamination at intervals not to exceed six months.
Also, License Condition 13.B requires that records of leak test results shall be maintained for inspection by the comission.
Contrary to the above requirements, at the time of the inspection leak test records for the Portaprobe Serial No. C76051695 containing a.10 millicurie cesium-137 and a 50 millicurie americium-241 sealed source were not maintained.
This is a Severity Level VI Violation.
