Appendix A NOTICE OF VIOLATION Hutchinson Area Vocational License No. 22-15554-01 Technical Institute As a result of the inspection conducted on July 8, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified:
License Condition No. 17 states, in part, that the licensee shall possess and usc licensed material in accordance with statements, representations and proceudres contained in application dated Spetember 30, 1980 which specifies in Paragraph 2.3 that individuals wishing to become radiographers will satisfactorily complete your radiographer's examination.
Contrary to the above, Mr. Michael Engblom has not received this examination since August-20, 1980 and has acted as a radiographer since that date.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1)- corrective cetion taken and the results achieved; (2) corrective action to be taken to avoid further noncempliance; and fo) the date when full compliance will be achieved. Under the authoritj i Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted
.under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
