Enclosed please find a copy of my original licensing amendment request concerning a minor change to Certificate of Compliance No. 9022, Rev. 5 (Model CE-250-2 shipping container). Attached to the request is an engineering justification analysis in support of the amendment request.
Also enclosed is a check for $150 to cover licensing fee charges (minor licensing change).

Therefore, an administrative amendment fee of $150 is required, as specified in fee Category llD of Section 170.31 of Part 170, copy enclosed.
Payment should be made to the U.S. Nuclear Regulatory Commission and mailed to my attention.
