3 Appendix A NOTICE OF VIOLATION Cincinnati Gas and Electric Docket No. 50-358 As a result of the inspection conducted on May 1, 4-8, 11, 18-22, and 26-29, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion V requires in part:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accord-ance with these instructions, procedures, or drawings."
The CG6E Company Construction Quality Assurance Manual, Section 5, paragraph 5.1 states in part:
" Construction, fabrication, and manufacturing activities which affect the quality of the facility are accomplished in accordance with written instructions, procedures, and drawings which prescribe acceptabic methods of carrying out those activities."
Contrary to the above, violations of procedural requirements were identified for the following procedures:
(a)
SU.PRP.12, Revision 00, paragraph 6.3; I&C technicians were assigned responsibility for hanging green jurisdictional tags although the procedure specifies that tags be hung by a rad / chem technician or a plant operator. Also, the I&C foreman was not aware of the requirement that the turnover coordination group witness the tag hanging.
(b)
QA. SAD.07, Revision 02, paragraph 5.2.3.A; no QC inspection requirements were identified on the work request and no QC inspection plan had been prepared for nonroutine maintenance being performed on the "A" diesel generator.
This is a Severity Level VI Violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explantion in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
