This refers to the routine safety inspection conducted by Dr. W. J. Adam of this office on May 27, 1981, of activities at St. Vincent Hospital authorized by NRC Byproduct Material Licenses No. 48-03220-02 and No. 48-03220-03 and to the discussion of cur findings with you and Mr. Ron Sheele at the conclusion of the f.nspection.
The inupection was an examination of activities conducted under your license as they relate to radiation sr.fety and to compliance with the Commission's rules and regulations ar.d with the conditions of your license. The inspection consisted cf a selective examination of procedures and representative recor1s, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities conducted under License No. 48-03220-03 appeared to be in noncompliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required. No items of noncom-pliance were identified under License No. 48-03220-02.
