,o JUL 9 1981 Carmi Township Hospital License No. 12-17074-01 ATTN:
Mr. Lyndell Dyer Administrator Plum and Webb Streets Carmi, IL 62821 Gentlemen:
This refers to the routine safety inspection conducted by Mr. W. P. Reichhold of this office on June 24, 1981, of activities authorized by NRC Byproduct Material License No. 12-17074-01 and to the discussion of our findings with you and Miss Glenna Wick, R.T. at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During the inspection our inspector expressed concern regarding your method used to perform daily constancy checks of your dose calibrator.
Appendix D, Section 2 of the medical licensing guide dated October 1980 requires that the daily constancy checks of the dose calibrator shall include a comparison between the actual value of a long lived source (such as cesium-137) and the daily reading of that source. Your dose calibrator does not have the settings which will allow you to make this comparison.
Consequently, in your reply please describe your corrective actions to ensure that your dose calibrator will be adjusted so it will be capable of a comparison between the actual value and the daily reading of your cesium-137 source.
l This inspection also included a review of the actions described in your letter dated March 6, 1978, regarding the apparent items of noncompliance identified during our Janaury 25, 1978, inspection. We have no further questions regard-ing this matter.
