Appendix A NOTICE OF VIOLATION Cyprus Fawaiian Cement Corporation License Nu. 53-18471-01 91-005 Kaomi Loop Ewa Beach, Hawaii 96706 As a result of the inspection conducted on May 6,1981, and in accordance with Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A.
10 CFR 20.207(a) states that, " licensed materials stored in an unrestricted area shall be secured from unauthorized removal from the place of storege."

_ controlled for radiation protection purposes and was accessible to unauthorized individuals.
This is a Severity Level 7 Violation (Supplement VII).
B.
License Condition 16 states that the licensee shall conduct a physical inven-tory every six months to account for all sealed sources received and possessed under the license.
Contrary to the above requirement, at the time of the inspection no inventory had been performed since receipt of licensed material on October 3,1980.
This is a Severity Level V Violation (Supplement VII).
C.
10 CFR 19.11(a) states each licensee shall post current copies of the following documents: (1) The regulations in this part and in Part 20 of this chapter, (2) the license and documents incorporated into the license by reference (3) the operating procedures, an: (4) any notices of violation.
Contrary to the above requirements, neither the required postings of 19.11(a) were present nor was the alternate posting authorized by 19.11(b) present.
This is a Severity Level V Violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, Cyprus Hawaiian Cement Corporation is hereby required to submit to this office within thirty days of the date of this notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration m;/ be given to extending your response time for good cause shown.
