This matter was discussed during a phone call with your Mr. J. B. Silvenvood and Mr. R. H. Leonard on April 29,1981, and again with Mr. R. H. Leonard on May 18, 1981. Specifically, what is needed at this point is for United Engineers and Constructors to confirm in writing that they will not only sample the Seabrook surveillance reports, but will also sample the WPPSS surveillance reports to assure generic coverage. R. Costello dated March at United Engineers & Constructors Inc. in Philadelphia, Pa. on February 23-26, 1981. Three Nonconformances were identified by your staff. Our response is attached including a restatement of the requirements and specific Nonconformances. The actions proposed to correct the Nonconformances and preclude recurrence are also stated; as well as, the expected completion dates. 1 Attachment 2 Docket No. 99900510/81-01 n UE&C StatementsJf Corrective Action Criterion V of Appendix 3 to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions. - procedures, or drawings, of a type appropriate to the circumstances and shan be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Nonconformances from these requirements and the UE&C Corrective Actions are as follows: WPPSS Quality Assurance Procedure QA-7-2 requires that the Vendor l A. Surveillance Representative af ter witnessing, performing or verifying the functions listed on the Vendor Surveillance Check Plan (VSCP) sign and date the appropriate blocks of the VSCP. Contrary to the above, VSCPs contained in Quality Control Vendor Surveillance Reports 91-CRO-11, 91-CRO-5, 91-CRO-4, 91-(4)-CRO-2, 140-(4)-EPH-1 a el 155-TAR-1 were not signed and dated as required by QA-7-2, but we ce initialed and dated. UE&C Corrective Action The following WPPSS Quality Assurance Procedure QA-7-2 is being changed. definition for " signed and dated" will be added to the procedure: " signed anddated means dated and stamped or initialed or full signature". This revision win be completed by June 2,1981. Seabrook Quality Assurance Procedure QA-7-2 states in part "The Supervising Engineer - Vendor Surveillance will establish Vendor Surveillance files, for B. Vendor files will all Purchase Orders within the scope of this procedure.... include copies of the following documents:... (b) Vendor Surveillance Check Plan." Contrary to the above, the vendor surveinance files did not have a Vendor Surveillance Check Plan for Quality Control Vendor Surveillance Report No. 2 l for Purchase Order 201-1, Colt Industries, and did not have e Section 3 of Vendor Surveillance Check Plan for Quality Control Vendor Surveillance Report No.105 for Purchase Order 248-1 (Dravo). UE&C Corrective Action The Vendor Surveinance Representatives have been contacted and asked to The Supervising Engineer - Vendor resubmit the missing check plans. United Engineers and Constructors (UE&C) procedure GEDP-0034 entitled, " Response to Audits, Corrective Action Requests and other Quality Assurance Reports," describes UE&C organizational responsibilities in responding to NRC inspection findings. The interfacing diagram in. Appendix A of this procedure defines the responsibility for establishing corrective action due dates on NRC audit findings as the Reliability and Quality Assurance Department. Follow up to assure the UE&C responses and ( irrective actions are completed as connaitted is also the responsibility of the Reliability and Quality Assurance Department. Contrary to this procedure, the Reliability and Quality Assurance Department did not assure that the consnitted completion date for corrective action it response to a previous NRC inspection finding was met. UE&C Corrective Action We have again reviewed UE&C's GEDP-0034 (referenced above). We judge the GEDP-0034 to adequately describe the responsibility assignment for follow-up to assure response and corrective action completion. Our own internal assessment agrees with your finding -- we did not fully implement this procedure. A UE&C Memorandum will be issued, no later than April 17, 1981, to Project Managers and those concerned with the implementation of this procedure. This Memorandum will draw their attention to the requirements of the GEDP-0034 and place emphasis on full compliance. Further, to assure action is taken as a result of the Memorandums, R&QA shall conduct a verification audit of the " file, log and follow-up" aspects of the GEDP-0034.
