This refers to the inspection conducted by Mr. C. J. Hale and Mr. J. R. Costello of this office on March 17-20, and April 6-10, 1981, of your facility at Chicago, I: :inois, and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
Areas examined during this inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During this inspection it was found that you failed to meet certain NRC require-ments. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please orovide us within 25 days of the date of this letter a written state-I ment containing, (1) a description of steps that have been or will be taken to correct these items, (2) a description of steps that have been or will be taken to prevent recurrence, and (3) the dates your corrective actions and preventive measures were or will be completed.
j We are particularly concerned about the conditions which necessitated the issuance of Item B in the enclosed Notice of Nanconfonnance.
The conditions l
identified indicate a significant weakness in a substantial portion of your quality assurance program that requires prompt and effective corrective action.
Your response to Item B should describe thoroughly the schedule for completing your actions and should be given a high priority. We will be con-ducting detailed inspections of the implementation of your planned actions.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this Tetter and the enclosed inspection report will be placed in the NRC's Public Document Room.
If the report contains any infonnation that you believe to be proprietary, it is l
necessary that you submit a written application to this office within 25 days 810702OO M l

of the date of this letter, requesting that such infonnation be withheld from public disclosure.
The application must include a full statement of the reasons why it is claimed that the infonnation is proprietary.
The application should be prepared so that any proprietary information identified is contained in an enclosure to the application, since the application without the enclosure will.
