NOTICE OF VIOLATION As a result of the inspection conducted during the period of January 13-15, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified:
10 CFR 71.5 requires that shipments of licensed materials be made in comoliance with applicable regulations of the Department of Transportation.
49 CFR 172.203 (d)(ii) and (iii) require that a descriptic of the physical and chemical form of the material being shipped and the ~
activity contained in each package be specified on the shipping papers.
Contrary to this requirement, a shipment of radioactive material (Item No. 80-76) shipped from the ANO site on Decemoer 17, 1980, contained materials and activities that were not described on the shipping papers issued for the shipment.
This is a Severity Level IV Violation (Supplement V.D.).

RESPONSE
The shipment of radioactive material was returned on or about December 29, 1980 at which time it was inscected and surveyed.
The Radioactive Shipment Record was revised to properly describe the contents of tne snipment.
Procedure 1602.65, " Radiological Control and Handling of LSA Containers" was issued February 12, 1981, and establisnes the requirement for~ the inspection, survey and documentation of the container and its contents and further requires the Radioactive Waste Coordinator to maintain a Radioactive Waste Control Log. The Radioactive Shipment Record is prepared by the Radioactive Waste Coordinator from data contained on the Control Log and shipment documentation is verified by the Radioactive Waste Coordinator prior to departure.
Full compliance has been achieved.
