'O APPENDIX A NOTICE OF VIOLATION Aerotest Operations, Inc.
Docket No. 50-228 3455 Fostoria Way License No. R-98 San Ramon, California 94583 As a result of an inspection conducted on February 11, 12, 17, 24 and 25, 1981 and in accordance with the Interim Enforcement Policy 45 FR 66754 (October 7,1980), the following violation was identified.
1) 10 CFR 20.401(a) requires each licensee on a quarterly basis to record on a form NRC-5 or equivalent, the radiation exposure for all individuals for whom personnel monitoring is required pursuant to 10 CFR 20.202.
Contrary to the above, the neutron exposures received by two individuals for the period 1970 through February 1981 were not maintained or recorded on a form NRC-5 or equivalent form, as required by 10 CFR 20.401(a).
This is a Severity Level V Violation, Supplement IV.
