i 21 - Reporting of Defects and Noncompliance During AW's internal investigation relative to the 10 CFR 21 filed by ITT-General Controls, (NRC Bulletin 80-09),

AW discovered thirteen (13) actuators on the Wm. H. Zimmer job, AW order No. 90083, which.*ere " Lock Nut M'ounted" as opposed to the seismically qualified (4) Bolt Mounting on a 5 inch bolt circle.

AW has since this finding notified the Engineer (Sargent and Lundy) and Owner (Cincinnati Gas & Electric) of the situation.

Along with the notification AW submitted procedures detailing the corrective action required. As soon as these procedures are approved the subject units can be corrected as the required drilling fixture is presently on hand at AW.

Notes:

1) Upon discovery of this situation AW double checked all shipped jobs that had ITT's and did not find any other shipped jobs with ITT's and requiring l

seismic qualifications which had the l

ITT's mounted as such.

The Wm. H. Zimmer job, AW order No. 90083 is the only job affected.
