1.

The licensee shall obtain all infomation from the foreign exporter g;

necessary to complete the shipper's portion of Form NRC-741, " Nuclear y$ W Material Transactions Report."

If the licensee is the receiver of the material, he shaU record both the shipper's and receiver's information p'

on the Form NRC 741 and transmit pursuant to Section 70.54 of 10 CFR N

Part 70.

If the licensee is not the receiver of the material, he shall j

provide the shipper's infomation to the receiver of the material for b

inclusion on the receiver's Form NRC-741.

The licensee shall notify the appropriate regional Safeguards Branch,

Office of Inspection and Enforcement, Nuclear Regulatory Commission,

immediately, by telephone or telegram, if entry of any import shipment

authorized by this license is refused by the U.S. Customs Service.

3.

Each time material is imported, the licensee shall show the original y ~

of the license to U.S. Customs as authority for the import.
