Reolv to Notice of Violation Combined NRC Inspection 50-334/89-22 and 50-412/89-21 Letter dated January 11, 1990 Violation (Severity Level IV, Supplement I)
Descriotion of Violation (50-334/89-22-01)
Technical Specification 6.8 requires that written proc?dures be established, implemented and maintained covering activities listed in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Site Administrative Procedure 41, clearance Procedure Section VI.C.6.d.8 requires that the applicable control room status board print be updated following the posting of a clearance.
7 Contrary to the above, on November 9, 1989, the applicable control room status board print was not updated following the posting of Clearance No.
562735.
-This led to the loss of one required train of the Cold Overpressure Protection System.
Corrective Action Taken Upon discovery, immediate corrective action was taken to restore the
. inoperable train of the Cold Overpressure Protection System (OPPS) to service.
A review of the circumstances surrounding this event was conducted.
As a
result, the operator who failed to update the valve status boards has been counseled.
In
- addition, we determined that improvements were needed in the program for updating control room status boards.
Action Taken to Prevent Recurrence Operations is currently strengthening its program to insure status boards are updated during shift manipulations.
This program will define accountability for reviewing the shift manipulations and verifying the accuracy of the valve status boards.
The program is expected to be
- complete, governed by administrative procedures, and implemented by May 31, 1990.
Periodic internal reviews will be continued and included in the strengthened program to confirm the effectiveness of the corrective l
action.
Operating Surveillance Test (OST) 1.6.8 was revised to include steps to verify that the OPPS pressure transmitters are properly aligned.
Date of Full Comoliance The station is in full compliance at this time.
As discussed above, program enhancements should be implemented by May 31, 1990.
