.AE00 has taken the initiative to produce a video tape on " Good Practices in Preparing and Administering Radiopharmaceuticals." The intent of the tape is 1

corrective actions-to prevent misadministrations and other errors that have occurred in nuclear medicine-departments and have been recommended to NRC by I

Second Building Status The~ Director, Office of Consolidation, provided a revised estimate to General Services Administration (GSA) of the anticipated added costs to the JNRC that would result from acquisition of space near the Twinbrook Metrorail r

station rather than adjacent to the first building.

GSA is currentlyJreviewing the economic and other implications of three options: 1) lease of Two White Flint North (TWFN) to be built by the owner of the. property adjacent to One White Flint North (OWFN); 2) direct purchase of the lot and construction plans for TWFN which would then be built under Federal auspices; and 3) lease of a second building to be constructed near but not. adjacent to OWFN. Sites being considered include properties to be developed near the Twinbrook and Rockville Metrorail stations.

Comanche Peak Steam Electric Station, Units 1 and 2 1.

In March 1989, TU Electric entered into an agreement with Tex-La Electric Cooperative of Texas, Inc. -(Tex-La) under which TV Electric would purchase Tex-La's 2.17-percent ownership interest in the Comanche Peak Electric Station (CPSES). The staff issued amendments to the construction permits on. August 29, 1989 authorizing this transfer. The transfer of Tex-La's ownership interest to TV Electric was-completed on February 1,1990 and Tex-La is no longer listed as an owner of the CPSES.

I Until the completion of the 6.2-percent ownership interest transfer from Texas Municipal Power Agency (TMPA) to TV Electric, TMPA is still listed as an applicant. With the completion of the TMPA transfer (which is takingplacein~10 installments),TUElectric'saggregateinterestin CPSES will'be 100 percent.

On February 6, 1990, the Fifth Circuit of the U. S. Court of Appeals denied

a request by the Citizens for Fair Utility Regulation (CFUR) to stay the i

issuance.of the low-power license-for Comanche Peak Unit 1.

Although the

later, the stay denial did indicate that CFUR had not presented a compel-ling cu e for late intervention.

On February 6, 1990, the Citizens Association for Sound Energy (CASE) transmitted a request for "immediate review" pursuant to 10 CFR 2.206.

At approximately 4:00 pm on Thursday, February 8,1990, Dr. Murley signed license No. NPF-28 for Comanche Peak Unit 1.'

This license authorizes Texas Utilities Electric Company to load fuel and conduct low-power-testing at core power levels not in excess of 170 megawatts thermal-(5%'of rated power). The licensee expects that initial criticality 1

will occur about 50 days after license issuance (approximately March 30, 1990) i Zion, Unit 1 At approximately 7 PM on January 23, 1990, the licensee discovered a manual, i

locked auxiliary feedwater-(AFW) va1ve misaligned (closed when it should have

-beenopen)during a performance test on the IB AFE pump. The unit was in l

' Mode 2 at two percent power in the process of starting up following a f

refueling outage'. The misalignment would have prevented flow from the IB pump i

'to any steam generator had AFW been initiated. The recirculation line was i

operable and-prevented damage to the pump.

It is-believed that the valve misalignment had existed since ap)roximately 12:40_PM the same day when the system was realigned to return t1e 1A AFW pump t-

' on' January 16-due to another valve misalignment. The operator and the independent. verifier (a shift foreman) both assumed the valve was open based on its stem-position and did not attempt to physically turn the valve to u

L verify its position as taught in the operator-training program. The valve had been replaced during this refueling outage and had a different length stem than a similar valve nearby.

Oconee Nuclear Station, Units 1, 2, and 3 On January 29, 1990, a Materials License was issued to Duke Power Company to

-operateanindependentspentfuelstorageinstallation(ISFSI)locatedatthe Oconee Nuclear Station site. The ISFSI will provide dry storage in horizontal storage canisters for spent fuel from Oconee Units 1, 2, and 3.

TMI-2 reactor vessel lower head was vacuumed and placed in a fuel canister.

A; subsequent' video exam of the reactor vessel confirmed that all accessible debris was removed. -The licensee states that this represents the end of_

fuel removal.

Following the confirmatory video, the NRC's Office of Nuclear Regulatory Re-L search began taking' metallurgical samples from the reactor vessel-lower head.

'This effort is.in cooperation with the Organization for Economic Cooperation

Land-Development (ECD) and will last approximately one month.

Parallel to and following the research program, the fuel ccnisters must be dewatered, weighed, l transferred to the fuel handling-building, and loaded into shipping casks.-The casks will be shipped to the _ Idaho National. Engineering Laboratory in the '.

March / April time frame.

On Tuesday, February 6,1990, a combined work crew from General Public Utilities E

Nuclear (GPUN), MPR Associates, and Power Cutting Incorporated (PCI) obtained the first " boat" sample from the TMI-2 reactor vessel lower head. The sample-was obtained from the K-7 grid location area. A previous attempt _to sample the'H-7 location was tem)orarily abandoned with a broken electrode from the L

metal disintegration mac11ne remaining in the cut. The combined work crew was h

in the process of cutting a second boat sample at the F-10 grid location on l

Wednesday, February:7. The overall sampling program goals are to obtain

.between 8 and 20' samples from the lower head as well as portions of incore t

l l-instrument penetrations and incore instrument guide tubes.

In an unrelated matter, a shaft on the main com)ressor of the water evaporation system failed on Saturday, February 3,1990. Tle evaporator system will be used to disposeLof the " accident generated water" resulting from the TMI-2

- accident.. The system.is currently in the startup and. test phase, using a L

non-radioactive. surrogate test solution. The repair to-the shaft of the La Roots-type compressor is expected to'take approximately _3 weeks. This would place the earliest expected operation of the. evaporator system in mid-April.

f NCRP Report on Hot Particles Mr. Beckner, NCRP, hand delivered galley proof copies of Report No. 106, " Limits L for Exposure to ' Hot Particles' on the Skin" to NRR, RES, and the Comissioner's staff on February 7,1990. The staff will review the report and evaluate its effect, if any, on the Commission Paper (SECY-89-370) addressing this topic.

, proposed license amendment.. B&W, which has constructed a building to house a supercompacter for contaminated waste, intends to request authorization to operate the compactor.

Louisiana Energy Services' Fuel Cycle Safety Branch staff visited the proposed site of Louisiana Energy Services (LES) Uranium Enrichment Plant in. Homer, Louisiana, on February 5 t

6 1990, and then met in Baton-Rouge the following day with Michael McDaniel Assistant Secretary of-the Louisiana Department of Environmcntal Quality IDEQ),

members of his staff, and' Region IV representatives. This meeting was requested

' status.of: the LES-project.and to answer questions relating to local citizens' concerns and licensing procedures, pending Congressional legislation,-and l

HIGH-LEVEL WASTE MANAGEMENT Nuclear Waste Technical Review Board Meeting On January 31 and February 1 - 1990', representatives of the Division-of High-Level Waste Management-(HLWM. attended a Nuclear' Waste Technical-Review Board 1

(TRB) neeting. The_ purpose of the TRB meeting was'to have the U.S. Department of Energy (DOE) discuss its plans for conducting alternative exploratory shaft

._ facility (ESF) design studies and to describe the approach it plans to use to sprioritizo surface-based site characterization work for'early identification of any site disqualifying' conditions. Based on the presentations, the TRB

endorsed the DOE approach to conducting the processes under a quality assurance

'(QA)-program. This panel'of the TRB agreed to meet again to discuss the ESF alternatives study on-April-5, 1990.

> LOW-LEVEL WASTE' MANAGEMENT Meeting'with Washington State on Petition for Rulemaking

.0n February 7,1990, the staff met with representatives of the State of Washington to discuss the State's Petition for Rulemaking concerning the classification 'of Hanford tank wastes. The discussion focused on the requirements (Section 2.802) for submitting a petition. Based on the discussion in this meeting, the State intends to revise and resubmit their petition.

Meeting with Kerr-McGee on the Cushing, OK and Cimarron Sites On february 7,1990, Low-Level Waste Management (LLWM) and Industrial and Med(0 1.5 clear Safety (IMNS). staff met with representatives of the Kerr-McGee Corporation to discuss the status of site investigations at the Cimarrotrf4?ility and the Cushing site. There are a number of site investV;ttion questions that were raised by the staff regarding the groundwater conditions at the Cimarron site. Kerr-McGee's geological consultant will meet with the technical staff on February 22 to go over these issues. -Kerr-McGee was asked to submit a letter describing their plans and schedules for cleanup of radioactive material at the Cushing site, j

SAFEGUARDS Domestic Implementation of Comparability Upgrade Requirements Staff has completed its review and approval of. Category I licensee physical security plans submitted in response to the Comparability Upgrade Rule.

-International TechnicalSupportCoordinatingCommittee(TSCC) Meeting On Tuesday, February 6,1990, a member of the International Safeguards Branch participated in a meeting of the TSCC, the U.S. Government Interagency Committee responsible for administering the U.S. Support Program for Safeguards to the International Atomic Energy Agw;cy (IAEA). Principal topics of discussion included:

the 1990 program status and response to ItFA TSCC budget for 1990, status of specific tasks, and export arrangements for safeguards equipment.

Transportation Plutonium Air Shipment the staff met with representatives of Lawrence Livermore 21,1990,(LLNL),toreviewtheprogressinthedevelopmentcfdraft On January National Laboratory

' test criteria for plutonium air transport packages to satisfy the requirements i

of Public Law 100-203 (Murkowski Amendment). The final d hft report including the controlled test criteria is due in September 1990. The controlled test criteria are alternate test conditions conducted on a package in lieu of conducting an actual air crash. The project is on schedule and within budget.

On February 7 IRB arovided training to seven members of the Department of State and to members of tie Executive Team in preparation of their participation in the forthcoming Fermi exercise on February 14.

On February 7 an IRB staff member attended a meating of the Training Subcommittee of the Federal Radiological Preparedness Coordinating Comittee (FRPCC). A major topic of discussion was the cancellation by FEMA of the Radiological Emergency Response and Operator's Course given at the Nevada test site. DOE intends to continue to offer this course on a pay-as-you-go basis.

On February 9, the AE0D Director, DOA and DSP Division Directors and IRB Chief met with a Department of Energy (DOE) senior manager to provide solicited infor-mation and advice regarding a new DOE group on " Nuclear Self-Assessment and Emergency Management." DOE staff will observe the Fermi exercise.

On February 9, the AEOD Director and IRB Chief provided participants in the InstituteofNuclearPowerOperations(INPO)seniornuclearplantmanagement course with a tour of the Operations Center and a discussion of NRC incident response activities.

On February 8, the AEOD and TTC Directors briefed the Advisory Committee on ReactorSafeguards(ACRS)onNRCTechnicalTrainingPrograms. The briefing i

updated the ACRS on technical training initiatives since the last briefing including the following: development of headquarters technical training requirements, the ongoing review of regional reactor inspector training require-ments, implementation and consolidation of the health physics curriculum, expanding risk-based perspectives in TTC reactor technology courses, the simulator upgrade plan, and high technology training enhancements.

AEOD has taken the initiative to produce a video tape on " Good Practices in Preparing and Administering Radiopharmaceuticals." The intent of the tape is to stress in a positive way to NRC licensees standards of good practices in l

nuclear medicine. These practices have been suggested by NRC licensees as corrective actions to prevent misadministrations and other errors that have l

Meetings with Louisiana Officials on Emergency Preparedness and Enrichment Facility Requirements Charles Hackney, Region IV State Liaison Officer, met with Robert Warren, Assistant Secretary of the Louisiana Office of Emergency Preparedness, on February 5,1990 in Baton Rouge Louisiana. The purpose of the meeting was to discuss emergency preparedness Issues and to review the Office of Emergency Preparedness organization, staffing and training.

Charles Hackney attended a meeting with NMSS staff and officials from Louisiana on February 6, 1990 in Baton Rouge, Louisiana. The purpose of the meeting was to discuss NRC licensing requirements for enrichment facilities; to brief the State on the status of pending uranium enrichment legislation and to review the schedule of the proposed Louisiana Energy Services enrichment project.

1 Utah Radiation Control Program Review Reviews were conducted of the Utah and Illinois Radiation Control Programs this week. Preliminary staff findings will be reviewed by Regional and Headquarters offices and the results will be confirmed in a letter to the State.-

Peach Pottom Emergency Preparedness Exercise Marie Miller, Region I State Liaison Officer, participated in an emergency preparedness exercise for the Peach Bottom Atomic Power Station on February 7, 1990. Ms. Miller acted as a Federal representative at the State Emergency Operations Center in Harrisburg, Pennsylvania, j

International Programs Exchange of Assignees with Japan On Tuesday, Kazuhiko Hcmbu, Senior Deputy Director, General Coordination Division, Agency of Natural Resources and Energy, Japan Ministry of Inter-national Trade and Industry (MITI), met with Ron Hauber and Kevin Burke to discuss placement of MITI engineers at NRC (currently there are two MITI assignees working in NRR). There was also a discussion of the possible assignment of an NRC engineer to work in Japan for six to twelve months with MITI. Each side has agreed to continue to explore the feasibility of this assignment.

Upcoming Visit of Korean Delegation Arrangements are being finalized for the February 20-22 visit to NRC of a ofScienceandTechnology(MOST) four-man delegation from the Korea Ministry (the former Nuclear Safety Center of and the Korea Institute of Nuclear Safety the Korea Advanced Energy Research Institute, which will become Korea's inde-pendent regulatory body effective February 15,1990). The team, headed by Mr.

Seong-Bin Hong, Director of the Nuclear Safety and Cooperation Office of tiOST, will be visiting the U.S. and Canada as part of the Korean Government's effort to study ways to increase public education and public participation in the licensing process (particularly in the public hearing and review areas), as well as how to institute a fee system to help recover regulatory costs. The group has been scheduled to meet with GPA, OGC, the ASLBP, the ASLAP, the ACRS, and OC during its three days of appointments at NRC.

e Office of Enforcement Items of Interest Week Ending February 9,1990 Significant Enforcement Action A Notice of Violation and Proposeo Imposition of Civil Penalty in the amount of $500 was issued on February 1, 1990 to Buttu-Silver Bow, Department of Public Works, Butte, Montana. The action was based on licensee's failure to provide personnel dosimetry to gauge users, leak-test sealed sources, perform inventories at 6 month intervals, prepare shipping papers and have certificates required showing special form and Specification 7A testing and possession of an unauthorized gauge, storage of a gauge in an unauthorized location and having an unauthorized individual as radiation protection officer. These violations are of concern as they indicate in the aggregate, a lack of management oversight of the licensee's radiation safety program. The mitigation / escalation factors in the Enforcement Policy were considered and no adjustment to the base civil pencity was considered appropriate.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $7,500 was issued on February 2,1990 to Georgia Power Company (Vogtle).

The action was based on the repetitive failure on the part of licensee employees to adequately protect and account for documents containing safeguards informa-tion. The base amount of the civil penalty for this Severity Level IV violation was mitigated by 50% due to the licensee's identification and reporting of this most recent violation.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $1,250 was issued February 7,1990 to Ivinson Memorial Hospital, Laramie, Wyoming. This action is based on licensee's failures:

to have the Radiation Safety Committee (RSC) meet in L e second quarter of 1988, to have the RSC perform an annual review of the radiation safety program in 1988, to perform leak-tests each 6 months between September 10, 1986, to October 26, 1989, to calibrate survey instruments, to perform all steps required for dose calibrator constancy and linearity testing, to perform a dose calibrator accuracy test for 1988, and maintain records of waste disposed of through decay-in-storage.

These violations are of concern as they indicate in the aggregate, a lack of management oversight of the licensee's radiation safety program. The mitigation / escalation factors in the Enforcement Policy were considered and the base civil penalty reduced by 50 percent because of the licensee's prompt and extensive corrective actions.

Civil Penalty Paid TV Electric (Comanche Peak) paid the civil penalty in the amount $30,000. The action was based on multiple violations identified by an AIT after the applicant experienced a backflow event through Auxiliary Feedwater system check valves on April.23, 1989. The violations involved two instances of operator failure to follow system lineup procedures during hot functional testing and four examples of inadequate corrective actions for identified deficiencies. Had these inci-dents occurred during plant operation, they would likely have constituted a

However, because substantial construction activities were still under-way during the conduct of the hot functional testing, it was concluded that these events should be judged :rgainst the examples in Supplement II, Part 50 Facility Construction, of the Enforcement Policy. The violations cited do not appear, even in the aggregate, to fit the examples for a Severity Level !!!

Week Ending February 9,1990 Second Building Status The Director, Office of Consolidation, provided a revised estimate to GeneralServicesAdministration(GSA)oftheanticipatedaddedcoststothe NRC that would result from acquisition of space near the Twinbrook Metrorail station rather than adjacent to the first building.

GSA is currently reviewing the economic and other implications of three options: 1) lease of Two White Flint North (TWFN) to be built by the owner of the property adjacent to One White Flint North (OWFN); 2) direct purchase of the lot and construction plans for TWFN which would then be built under Federal auspices; and 3) lease of a second building to be constructed near but not adjacent to OWFN. Sites being considered include properties to be developed near the Twinbrook and Rockville Metrorail stations.

OWFN Cafeteria On Thursday, February 8, the Director and members of the Office of Consolidation, met with the Director, Vending Facilities Division of Blind Industries and Services of Maryluid (BISM), the Chief of GSA's Concessions

. Branch, and representatives of NRC's Facility Management Branch and Division of Security to discuss the hours and scope of operations of the OWFN cafeteria and related issues.

The current target date for opening the cafeteria for business is early April.

Linda Edwards', who currently operates the brown-bag area blind stand on the second floor of OWFN, is the proposed blind vendor for the cafeteria. She plans a service from 6:45 a.m. until 4:00 p.m. Monday thrcugh Thursday and from 6:45 a.m. to 2:30 p.m. on Friday. This schedule may change later depending on patronage.

that they were planning to add two additional tracks to their rail mar-shalling yard in Erwin, Tennessee. The railroad's proposal involves placing two tracks across property currently leased to NFS. The property in question contains two radioactive waste burial trenches which were o)ened in 1969 and closed in the early 1970s. (RBS) declared that all c1111ers in both divisions of the Control Building Chilled Water System were technically inoperable. The licensee notified the Region IV management, at which time a request was made for a temporary waiver of compliance from the site's technical specification (TS). The

Rancho Seco Nuclear Generating Station a.

On February 1, 1990, the licensee informed the NRC cf an event which 1

occurred on January 27, 1990, in which the licensee inadvertently-1 transferred 20,000 gallons of borated water from the spent fuel pool to the borated water storage tank and lowered the water level in the spent fuel pool by two feet over a two-hour period. The cause of the transfer was due to the misalignment of valves when personnel operated the spent fuel pool demineralizer system. The demineralizer system removed water from the spent fuel pool and due to a valve.

The licensee has contracted with Saul Levy and Associates to conduct-a study on the feasibility in shipment of spent fuel from Rancho Seco to another licensee for interim storage.

recommendations on containment design criteria for future reactors in the spring of 1990.

The recommendations will discuss defense in depth policies and outline the issues and criteria for containments based on present knowledge.

Chairman Carr informed the ACRS that the Commission will respond, in the near term, to the ACRS letter on the Systematic

' Assessmant of Licensee Performance (SALP) process.

The Commission encourages the ACRS to continue providing its perspective and advice on the wide range of safety issues facing the agency.

The Commission requests that ACRS, in scheduling its activities, give priority to the review of reactor operational safety issues, technical requirements for future reactor designs and the technical criteria for license renewal.

On issues such as reactor aging, the Committee should structure reviews involving many disciplines in such a way that the advice to the Commission is well integrated.
