4 This letter addresses ~ the inspection of your facility at Mount Vernon, Illinois led by Mr.- J. J. Petrosino on October 3-7, 1988, and the discussions of their findings ^ with you and other members of your staff at the conclusion of the

. inspection. Release.of this report was delayed during NRC's ongoing review of nonconforming and substandard vendor products.

The purpose of this inspection was to review the methods in which you are-qualifying electrical circuit breakers for use in safety-related systems, as.

discussedinNRCInformationNotice(IN)88-46. The inspectors also evaluated your corrective actions in. response to our previous inspection report No.

99900779/87-01.

I understand that you have taken a proactive approach in resolving our 1987 concerns and have established many new program improvements.

LI urge you to continue with your program enhancements. Areas examined during our inspection and the findings are discussed in the enclosed report. This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.

During this ins assurance (QA) pection it was found that the implementation of your quality program failed to meet certain NRC requirements.

It was identified' that your commercialgrade(CG)programforthededicationandqualificationofcircuit breakers (CB) failed dedication basis (in cases where you use the similarity analyses method for CGCB upgrading) without having traceability of the individual CBs back to its original equipment manufacturer. Additionally, certain of your program activities that were found deficient during the previous inspection still require additional monitoring and surveillance. The specific findings and references to the pertinent requirements are identified in the enclosures to this~ letter..

Please provide us within 30 days from the date of this letter a written statement containing:

(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. We will consider. extending the response time if you show good cause for us to do so.
