Dear Mr. Hunt:

This letter addresses the inspection of your facility at Bath, England

~

conducted by. Messrs. Edward Baker and Uldis Potapovs on October 9-10, 1989,

-and the discussion of their findings with you and other members of your staff at the conclusion of the inspection.

The,) purpose of the inspection-was to review portions of your Quality Assurance' s

(QA program related to the. production.and supply of actuator parts and

-actuators to the commercial nuclear industry in the United States of America.-

During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements that were imposed on Rotork Controls,

>: Limited by contract.- While the inspectors found that assembly.ard testing-o activities conducted in your facility were well controlled, the inspectors determined that Rotork's QA program was not adequately controlling subcon-tractor's. performance.

Rotork's QA program does not assure that _ subcontractors

.are ' effectively implementing their QA programs. This has resulted in problems-with nonconformino or indeterminate hardware, drive pins, wormshafts, 0-rings, and, terminal blocks.. These issues are discussed in more detail in the enclosed report.

During this inspection it was found that:the implementation of your.QA program i

failed to meet certain NRC requirements. The specific findings and references

Please provide us within 30 days from the date of your receipt of this letter a 0

written statement containing:

(1) a description of steps that have been or

,will be taken to correct these items; (2) a description of steps that have been l=,

or will be.taken to prevent recurrence; and (3) the dates your corrective _

' actions and preventive measures'were or will be-completed. We will consider 1

extending the response time if you can show good cause for us to do so.

I The-response requested by this letter is not subject.to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
