Borger, Texas License: General' License (10 CFR-150.20)-
During a' review of your activities, a violation of~NRC requirements was-identified.
In'accordance with the'" General Statement:of Policy and Procedure for NRC Enforcement Actions'," 10 CFR Part 2, Appendix C (1989):(Enforcement Policy), the violation-is listed below:
10 CFR 150.-20(b) requires, in part', that any person engaging'in activities in-non-Agreement States under.the generalt license prov.ided in
. Section 150.20 :shall file copies of NRC Form-241~, " Report of Proposed Activities ~ in Non-Agreement States," with the Nuclear Regulatory Commission.
Contrary to this requirement, the-licensee did not provide NRC with this.
notification for radiographic work'usir.g byproduct material conducted November 13-20, 1989, in the state of 0klahoma,_ a non-Agreement State.

Pursuant to the provisions of 10-CFR 2.201, Panhandle-NDT and1 Inspection; Inc.,
is hereby required to submit to this office, within = 30 days of the date aof the-letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps-which have been-taken and the issults achieved, (3) the corrective steps which will be taken to'avoic.further violations,~and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
