Dear Sir:

In accordance with the requiremente of Title 10, Chapter 1, Code of Federal Regulatione, Part 21, Energy Services Group, a division of Cooper Industries, hereby notifies the Commiecion.of a potential defect in a component of a DSR or DSRV Standby Diesel Generator cystem.

There existe a poten-tial problem with Starting A$r Receiver Relief Valve, a safety related component in the starting air system.

Cooper Industries eupplied DSR and DSRV engines and/or apare parts with this potential defect to the following sites:

Thie, according to CEI, was the result of the simultaneous loes of starting air to both receaver tanke.

CEI further reported that the etarting air re'Jief valve was inadvertently hit csucing the valve to lift ano relieve the tank.

The valve did not recent until at apprc.ximately 120 peig; 30 peig below the start signal lockout.

Cleveland Electric concluded that the condition was not reportable under 10CFR21 due to planned corrective action, but advised us for our further

Both probleme occurred during maintenanae activities.

The affected valvec are Crosby etyle JMBU and JRU relief valves when used in the starting air system.

While the valves are deeigned and manufactured by the Croeby Valve 'nd Gsuge Company, component selection and design specificatione were the responsibility of Traneamerica Delaval (now Energy Services Group, a divicion of Cooper Industries).

Our investigation of the problem confirmed that the original component celection resulted in a valve with blow-down reset of 40 - 00% of the preneure set point (standard for this design).

In retrospect, the design specification should have addreseed this characteristic, cpecifying a reset pressure that fit within all limitations of the engine start eequence.

This investigation was made more difficult because of related issues, i.e.,

eeiemic response, maintenance versue operational status, valve eeneitivity due to horizontal mounting and redundant / isolated air eupplies. We concluded, however, that no matter what conditions may cause the valve to lift and in spite of system redundancies, a reset pressure below the start signal lockout pressure le not acceptable.

Attachneet "A" to this letter shows the Enterprice part numbere affected and applicable sites.

Cooper'e recommendation for corrective action is to replace these valves.

We are in the procese of specifying a qualified replacement valve and will have specific informa-tion available by February 15, 1990.

In the interim, close surveillance of the starting air eyetem chould be maintained.

Caution chould be exercised when working in the vicinity of thie valve.
