4 NOTICE OF VIOLATION a'
Commonwealth Edison Company Docket No. 50-454 Docket No. 50-455 f
As a result of the inspection conducted from November 18 through December 30,.
1989, and in accordance with 10 CFR Part 2, Appendix C - General Statement W
of Policy and Procedure for NRC Enforcement Actions (1989), the following 3
. violation was identified:

.10 CFR 50, Appendix B, Criterion XVI, states that measures shall be established to assure conditions adverse to quality, such as deficiencies

are promptly identified and corrected.
L Contrary to the above, adequate measures had not been established to identify and correct in a timely manner deficiencies in test samples of grease obtained from environmentally qualified (EQ) motor operated. valves (MOVs).
'i Specifically, measures did not adequately exist to ensure _ a Deviation Report (DVR) was issued for discrepancies identified on January 6,1989, in grease samples for EQ MOVs ISI88048 and 2SI8814.
The grease was subsequently replaced with acceptable grease on February 8,1989 (2S18814) and July 28, 1989 (1518804B). Also,.since a DVR was not issued, the MOVs were not identified for review for. operability assessment by cognizant operating and technical staff personnel in a timely manner.-
This is a Severity Level IV violation (Supplement.1).

The inspection showed that action had been taken to correct the identified violation and to prevent recurrence.
Consequently, no reply to the violation
-is required and we have no further questions regarding this matter, at this time.
