ENCLOSURE TMI-1 AND TMI-2 STATUS REPORT FOR THE PERIOD OCTOBER 17 - NOVEMBER 27. 1989 1.
TMI-1 a.
Facility Operations Summary TMI Unit 1 operated at full power during this report period, completing 334 days of continuous operation on November 27, 1989.
On October 30, 1989, during performance of an Engineered Safeguards Actuation System Surveillance procedure, the plant experienced an inadvertent ESAS actuation.
The actuation was caused by personnel error. An off-shift operator, conducting the test, failed to perform the required verification steps as specified in the procedure. The emergency equipment responded as designed. Approximately 80 gallons of borated water from the borated water storage tank (BWST) was discharged to the reactor vessel via the high pressure injection (Hpl) system. The licensee made the required notifications in accordance with 10CFR 50.72, 2.
TMI-2 a.
Facility Activities Summary i
During this report period, defueling crews compleimi removal of baffle plates and defueling of the core formers. Defueling commenced in the lower core support assembly. Approximately 5,000 pounds of material remain in the vessel. Defueling was projected to be completed in early December.
Following completion of defueling, a number of samples will be taken of the lower head and incore instrumentation nozzles. These samples will be analyzed to assess the effects the accident had on the reactor vessel, b.
Items of Special Interest On November 22, 1989, an enforcement conference was held at the NRC Region I Headquarters. The conference dealt with the worker over-exposure event of September 25, 1989. The licensee presented the dose assessment ano corrective actions to the NRC staff.

NRC Staff Activities The NRC staff assigned on site during this period consisted of a senior resident inspector, three resident inspectors, and a project manager for TMI-2.

Inspection Reports During this period, Region I issued the following inspection reports.

included; an integrated control system transient, engineered safety feature walkdown of the building spray system, radiation monitoring system pump upgrade,. penetration pressurization valve modifications, allegation follow-up concerning licensed operator examinations, t
repair of makeup system weld joint, and licensee action on previous j
inspection findings.

--TMI-I Inspection Report 50-289/89-21 addressing the radiologicel environmental monitoring programs including: Management controls, audits, quality assurance, and implementation of the above programs.

--TMI-2 Inspection Report 50-320/89-09 addressing a reactive inspection to review the circumstances connected with an apparent overexposure 7
of two workers in excess of the limits specified in 10 CFR Part 20.

NRR Licensing Action _s On October 10, 1989, the NRC office of Nuclear Reactor Regulation (NRR) issued Recovery Operations Plan (ROP) change approval 42.
This change allows the licensee flexibility in cross-tieing of 480 volt electric busses without a loss of redundancy, c.
Monthly Status Report As the defueling activities at TMI-2 near completion, a monthly status report is no longer merited.
Licensee activities and the activities of the NRC staff will continue to be addressed in routine inspection reports. Accordingly, this is to advise readers that the monthly status report will discontinue once reactor vessel defueling is completed.

Public Meetings The next panel meeting will be held in January 1990.
The time, date and location will be announced via press releases and a future monthly status report.
