DETAILS OF INQUIRY

Purpose of Inquiry This Inquiry was initiated on March 4, 1987, at the. request of the _then Acting Executive Director for Operations, Victor STELLO, Jr., to determine if Telemecanique, Westminster, Maryland, was issuing) fraudulent Certificates of Conformance (C00) for Motor Control Centers (MCCs manufactured, assembled, and distributed by them (Exhibit 1).

Background
On November 1-9, 1984, NRC Vendor Branch Inspectors Kamalakar R. NAIDU and Joseph J. PETROSINO conducted an inspection of Telemecanique's quality assurance (QA). program. The inspection was conducted to ensure that the MCCs manufactured by Telemecanique and installed with circuit breakers (CBs) l manufactured by Siemens-Allis (SA), Bellefontaine, Ohio, met the 0A requirements of both 10 CFR 50, Appendix B, and ANSI N45.2 (Exhibit 2).
The Inspectors determined that Telemecanique's QA program. failed to meet

Criteria VII, X, and XVIII of Appendix B to 10 CFR 50, which requires vendors l
of IE equipment to assure that commercial grade material. purchased from thair sub-vendors that will be dedicated as IE, conform to procurement documents, 4
and that they establish a QA program to test circuit breakers received as commercial grade items.
