I APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No.
50-320 Three Mile Island Unit 2 License No. DPR-73 J
As a result of the inspection conducted on November 30 1989 and in accordancewiththeNRCEnforcementPolicy(10CFR2,AppendIxC),the following violation was identified:
be made such s(b) requires,-in part, that each licensee shall make or cause to 10 CFR 20.201 urveys as may be necessary and reasonable to-ensure compliance with the requirements of 10 CFR Part 20.
10 CFR 20.201(a) defines a survey, in part as an evaluation of the radiation hazards incident to the presence of radioacfive materials under a s)ecific set of conditions. When appropriate, such an evaluation includes.a p1ysical survey of material and equipment and measurements of levels of radiation present.
Contrary to the above, at approximately 2:15 a.m. on November 28, 1989'101-inadequate surveys were made to assure compliance with that part of 20 which limits radiation dose to indhiduals in restricted areas.
Specifically, i
a hose end-plug fitting was removed from the reactor vessel and handled by two technicians without properly measuring the radiation dose from the end-plug.
This resulted-in an unplanned radiation exposure to one of the technicians and in the inappropriate removal of contaminated material from the reactor vessel.
This violation is categorized as a Severity Level IV problem (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1 the corrective steps which have been taken and the results violations (2)) corrective steps which will be taken to avoid furt achieved; good:cause is sho(3) the date when full compliance will be achieved.wn, consider
