On-Site Review of OA Fire Pump Inoperable Greater Than Seven Days On-Site Review has investigated the inoperability of the OA Fire Pump.
-This Special Report was prepared pursuant to the requirements of Technical Specification 3.21.2.B.
1 On-Site Review has determined'the following:
On November 18,1989 at _0840 hours, the 0A motor-driven fire pump was taken out of~ service for the installation of an NFPA approved fire pump controller, per Modification M22-0-87-13.
The modification is being installed in:three segments.
Segment A consists of the irstallation of the pump controller and control switch and associated electrical equipment, segment B consists of the installation of a new'480 volt breaker into Bus 138, and segment C consists of the installation of pressure switch piping and' tubing on the fire pump.
Segment B of the modification was completed on November 21, 1989 and segment C on. November 24, 1989, however, segment A, the largest of the three, could not be completed within the seven day clock.

Specification 3.21.2.B. to restore the 0A Fire Pump to operability status within seven days, expired requiring this Special. Report.
Modification work on the 0A Fire Pump began on Saturday, November Ift, 1989.
The work is currently completed and the OA~ Fire Pump will.be declared
- operable as soon as post modification testing is completed.
This testing should be completed by December 22, 1989. 'A Vendor tecnnical representative, from Westinghouse Electric Corporation, has been present during the modification to provide assistance.
Two service water booster pumps (each rated at 4000 gpm capacity) provide water to the site main ring fire header.
Additionally, the OB Diesel-driven Fire Pump is a backup to the 0A Motor-driven Fire Pump.
The OB Diesel-driven Fire Pump automatically starts when system water pressure decreases to 110 psig, and the pump is rated at 2000 gpm at a discharge pressure of 141 psig.
During the time between 11-29-89 and 12-07-89, the OB diesel driven fire pump was' unavailable due to a supercharger failure. This is presently under investigation (DVR 22-1-89-169) and another report will follow.
In the event that neither service water booster pump nor the 09 Diesel-driven Fire Pump is available, the fire header will be fed from the m vice water pumps throuph a
. bypass around the service water booster pumps. Normally, pressure from the service water booster pumps holds a check valve in the bypass line closed.

are required.
