- Be advised that Limitorque has concluded its evaluation of data discovered in providing a valve actuator documentation package and concluded that there is a potential that equipment provided may not perform. as expected.
Therefore, pursuant to the requirements of 10 CFR Part 21, herein provided is notification of a defect in Limitorque's
- supplied actuators.
Identification Of Basic Component The equipment in question is an SMC-04 mounted on an HOBC, furnished on Limitorque's order number 3F1314-A.' Texas Utilities valve tag #X HV 5828 E and X-HV 5825E serial numbers M002310 and M002309.

Nature Of The Defect On performing the applications review for your documentation package, we identified a transposidon error in the actuator sizing calculations. The result is the available output torque'at the minimum voltage is less than that required to trip the torque switch at its minimum setting. This particular application is designed for position seating where the position limit switch controls the open and close positions of the valve and the torque switch acts purely as a overload protection device. Therefore, at the minimum voltage the valve and actuator would not have torque switch protection.
There is also a possibility that the actuator may not be capable of providing the required design torque at the minimum voltage condidon, however this determination could not be made because the valve manufacturer did not provide a torque requirement on his purchase order.
Eauioment Effected There are two actuators in question. Both are furnished on Limitorque's order number 3F1314-A with a model designation of SMC-04/HOBC.
Licensee Corrective Action Texas Utilities should determine the valve torque and operating time requirements and forward this information to Mr. Dan Warsing, Limitorque's Technical Manager.
Limitorque will perform an application review and provide alternatives for modifying the actuators to meet these requirements.
Limitoraue Corrective Action Limitorque has concluded that this error is an isolated case which was not detected by the independent review process. We have not experienced any similar occurrences.
