Dear Mr. Lewis:

This is in response to the notice of violation forwarded as Appendix A to the referenced report.

The five items that you require to be incorporated in my response are pre-sented below in the order specified in Appendix A.

1. Admission or denial of the alleoed violation I admit that required approval signatures were not obtained for work orders numbered 1751,1759,1796, and 1807 as required by procedure.

I wish to clarify that the work order procedure is not an area operating procedure as specified in the allegation but rather is a procedure set forth in Section 3 pf the demonstration o ction of our license application, specifically S 3.3.3.

2. Reason for the violation The individuals responsible for assuring that the approval signatures are entered were not sufficiently persistent in this effort. On several occasions attempts were made to locate the people designated to approve the work speci-fied on the form, but they were not conveniently available. After several unsuccessful attempts, the effort lagged and the forms were filed without the required signatures.
3. Corrective steps taken and results achieved The individuals responsible for obtaining the approval signatures on the work order forms have been reinstructed in this responsibility. The work order forms numbered 1751, 1759, 1796, and 1807 have been reviewed and approved by the individuals specified on each form. The Supervisor, Health and Safety, has been instructed to place the review of these forms as a check item on his monthly audit.
