APPENDIX A

~ NOTICE OF VIOLATION Northeast Nuclear Energy Company Millstone Unit 1 Hartford, Connecticut 06101 License No. DPR-21 Docket No. 50-245 As a result of the inspection conducted on November 5-7 and December 12-14, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754, October 7, 1980, the following two violations were identified:

A.

T.S. 6.11, " Radiation Protection Program", states: " Procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained, and adhered to for all operations involving personnel radiation exposure."

Station Procedure SHP 4902, " External Radiation Control and Dosimetry Issue", required each individual to wear the assigned personal dosimetry

devices during presence in any radiological controlled area (RCA).

Contrary to the above, at about 1 p.m. on November 5,1980, an individual was working in a RL4, the Reactor Building Equipment Access Lock, and l

was not wearing the assigned personal dosimetry devices, as observed by the inspector.

I This is a Severity Level IV Violation (Supplement IV).

Station Procedure SHP 4906, " Posting of Radiological Controlled Areas",

requires that all entrances to Radiological Controlled Areas (RCAs) be conspicuously posted with " Radiation Area" and " Radioactive Materials Area" signs.

Contrary to the above, an entrance to an outdoor RCA was not-posted and no other control was evident when -the inspector passed by at 8 ?0 AM and again at 8:45 AM on November 7,1980, and was not posted until the inspector then identified this item to the licensee.

This is a Severity Level IV Violation (Supplen.ent IV).

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:

(1) admission or denial of the alleged violations; (2) the reasons for the item (s) of noncompliance if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
