Mr. Clarence Snell President Post Office Box 470 Woodruff, Wisconsin 54568 Gentlemen:
This refers to the routine safety inspection conducted by Mr. J. R. Madera of this office on June 16, 1981, of activities at Howard Young Medical Center authorized by NRC License No. 48-18643-01 and to the discussion of our findings with Michael J. Thimmes-h, M.D. and Dan Spencer, R.T. at the conclusion of the inspection.
The inspcction was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.
The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of yottr activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
We will gladly discuss any questions you have concerning this inspection.
