Appendix A NOTICE OF VIOLATION Outer Drive Hospital License Nr. 21-13389-01 As a result of the inspection conducted on June 5, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violstions were identified:

1.

Condition No. 16 of your license requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in the referenced application and letter.

The referenced application dated August 14, 1979, states in Item 7, the Medical Isotope Committee will meet in accordance with Regulatory Guide 10.8, dated November 1, 1977, Appendix B.

Appendix B requires the Comnittee to meet no less than quarterly.

Contrary to the above, the Medical Isotope Committee did not meet from October 13, 1978, to July 13, 1979; from July 13, 1979, to November 4, 1980; nar from December 12, 1980, to the date of the inspection. These periods exceed three months.

This is a Severity Level V violation (Supplement VII).

Condition No. 16 of your license requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in the referenced application and letter.

The referenced application dated August 14, 1979, states in Item 10 that the dose calibrator will be calibrated in accordance with Regulatory Guide 10.8, dated November 1977, Appendix D.

Appendix D requires a lineari;.y check to be performed quarterly.

Contrary to the above, as indicated by licensee representatives' state-ments, linearity checks have not been performed as required since the license was renewed on October 4, 1979.

This is a Severity Level V violation (Supplement VII).

Condition No. 16 of your license requires that licensed aaterial be possessed and used in accordance with the statements, representations, and procedures contained in the referenced application and letter.

The referenced letter dated September 5, 1979, states survey meters will be calibrated annually or after repair.

Contrary to the above, a Picker CDV-700 survey meter used for area and package surveys was not calibrated from November 12, 1979, to March 18, 1981. This period exceeds one year.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance sill be achieved. Under the authority of Section 182 of the Atomic Energy Ac', of 1954, as amended, this response shall ba submitted under oath or affirmation.

Consideration may be given to extending your response time for good cause shown.
