General Atomic Company Docket No. 99900387/81-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on Marca 30 - April 3',
1981, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instruc-tions, precedures, or drawings, of a type appropriate to the circum-stances and shall be accomplished in accordance with these instructions, procedures or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances from these requirements are as follows:
A.
The Manufacturing Operations Manual, Revision B, Section 14, paragraph 4.1 states in part:
" Travelers are used to sequence operations required to fabricate and inspect parts and assemblies.
Each step in the fabrication process is described on the Traveler by an operation code.
Operations are seqt.entially performed, and when each operation is complete, the person who performed the operation signs and dates the step."
Contrary to the above the Manufacturing Operation Manual was not fully implemented in that 1) the sequence of operations was not followed in one traveler, 2) operations required to fabricate were not on another traveler, and 3) operations were not dated after performance in several additional travelers (See Details, paragraph C.3.b.(1)).
B.
Contract Specification Y2827, applicable to three current projects requires in Sections:
- 1) 313.1.c, nitrogen to be used for pneumatic tests, 2) STD-EF-242, electrical equipment if bolted to have the nuts welded to the skid base, 3) 1800-Z, motors to be capable of running at rated horsepower at - 10% rated voltage and to beeach channel to have at full torque at 75% rated voltage,
- 4) 302.17, 10% reproducibility at i 95% confidence limit.

Contrary to the above, there was not full conformance with contract requirements in that:
- 1) nitrogen was not used for pneumatic tests,
- 2) nuts were not welded to the skid base for electric equipment, 3) motor performance requirements were not required, and 4) reproducibility of channel signals was not documented.
C.
The Quality Manual, Revision F, Section 6, paragraph 2.0, states:
"The issuance of documents, including changes, shall be controlled to ensure that they are reviewed for adequacy, approved for release by authorized personnel, and are available for use at the location where the prescribed activity is performed."
Contrary to the above the issuance of some documents was not controlled, for example, an out of date Wire List 0358-2120-00 was in use although a new revision had been issued a week previously.
It was also noted that a record of the distribution of Wire Lists was not maintained therefore complete recall of out of date Wire Lists could not be assured.
