NOTICE OF VIOLATION Weldex Corporation License No. 04-04506-01 i

Industrial Radiographic Inspection 12223 Woodruff Avenue Downey, California 90241 As a result of the inspection conducted on May 26, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

A.

License Condition 16 requires that the licensee shall possess and use licensed material in accordance with statements, representations and procedures contained in the application dated June 11, 1974 and letters dated April 7,1976, December 1,1976 and February 28, 1977. Paragraph 2.5 of Section II of the Weldex Operating Procedures which is incorporated into the license application dated June 11, 1974 requires that management will exercise a continued review of quarterly inventories, utilization logs and surveys.

one daily maintenance and survey reports for May 1981 had not been reviewed and si7ned by the Radiation Safety Officer.

This is a Severity Level V Violation (Supplement VII).

B.

10 CFR 34.28(b) requires that records associated with the licensee's Maintenance and Inspection Program shall be kept for two years.

Contrary to this requirement, quarterly maintenance and inspection records for the second quarter of 1980 and the first quarter of 1981 were not maintained.

This is a Severity Level VI Violation (Supplement VII).

C.

10 CFR 34.26 requires that a record of inventory shall be maintained for two years for inspection by the Comission.

Contrary to this requirement, records of inventory for the second quarter of 1980 and the first quarter of 1981 were. not maintained.

This is a Severity Level VI Violation (Supplement VII).

D.

10 CFR 20.408(b) requires that a report of exposure incurred by an employee while employed by the licensee shall be sent to the Director of Management and Program Analysis, USNRC, within 90 days of temination of employment.

. Contrary to this requirement, the above mentioned report was not submitted for an employee who terminated on January 15, 1981.

This is a Severity Level VI Violation (Supplement VII).

10 CFR 20.409 requires that a report of exposure incurred by an employee while employed by the licensee shall be sent to the former employee at a time not later than the report to the Connission which is required by-10 CFR 20.408(b).

Contrary to this requirement, the above mentioned report was not sent to the former employee who was terminated on January 15, 1981.

This is a Severity Level VI Violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, Weldex Corporation of Downey, California, is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply. - - -- - --- -

including:

(1) the corrective steps which have been taken and the results achieved; (2) corrtictive steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response for good cause shown.
