Gescristico of Circumstances:
Ouring normal surveillance testing of motor-cceratec valves at the Ceccer Nuclear Station, it was discovered that :ne RHR "3" 1cco succression pcci cooling incoard throttle valve '.ould not ccerate.
The valve cic not coerate beca~. the key between the motor pinien gear anc :ne motor shaft had failed.
The ocerator for the valve has been identified as being Limit:rcue Operator Mocal_SMB-a.
Analysis of the failed key revealec that a standarc ty:e 1010 steel key had been installed instead of tne required special type 1130 steel key.
Further review of the problem establisned that similar failures with me:Or-to-shaft keys had also taken place at the ?iigrim, Haten, and Fit::atrick nuclear pcwer facilities.
Limitorcue C5mpany has performed an evaluatica of the problem, and they have concluded that the preolem is c:nfined to ccerators having a :crcue rating in excess of ICO ft/lb.
The standard type 1010 steel possesses adequate strength to acccmmccate motor ccerators having a torque rating -hat is less than 100 ft/lb.
