APPENDIX A NOTICE OF VIOLATION St._ Francis College-Docket No: 070-01050 Loretto, Pennsylvania 15940 License No:

SNM-1003 As a_ result of the inspection conducted on April 2, 1981, and in accordance with the Interim Enforcement policy, 45 FR 66754 (October 7, 1980), the following violations were 1dentified:

A.

Condition 16 of your license requires that licensed material be possessed and used la accordance with statements, representations, and procedures contained in your letter dated March 11, 1980.

Item 7 of this letter requires that a Universal Transistor Products Corporation Model Alphameter be available for monitoring and detection of radiation.

Contrary to this requirement, on the day of the inspection, April 2, 1981, the instrument described in your letter was not in your possession and it appears that neither this instrument, nor an equivalent one, has been available since 1975.

This is a Severity Level IV Violation (Supplement VII) 2.

Item 7 of this letter requires semi-annual calibration of radiation protection instruments.

Contrary to this requirement, on the day of the inspection, April 2, 1981, your radiar on protection instruments had not been calibrated since 1975.

This is a Severity Level IV Violation (Supplement VII).

Condition 13.B of your Licenu requires that leak tests made pursuant to Condition 13.A be capable of detecting the presence of 0.005 microcuries of radioactive material on the test sample.

Contrary to this requirement, as of the day of the inspection, April 2, 1981, leak tests performed since 1975 were not capable of detecting 0.005 microcuries of alpha radiation, in that the leak tests were evaluated with a Victoreen Model 6B GM survey meter attached to a probe with a 30 milligram per square centimeter window, a thickness which alpha particles can not penetrate.

This is a Severity Level V Violation (Supplement VII)

Pursuant to the provisions of 10 CFR 2.201, St. Francis College is hereby required to submit to this office within twenty-five days of the date of 8106190 W
