Description of Event

On May 22, 1981, while performing a routine walkdown, operations personnel noted that the expansion joint on the discharge of SAVS exhaust fan (1-HV-F-40A) had pulled loose from the ventilation ducting. Operation of the fan was terminated and maintenance personnel notified. Other expansion joints were examined with no discrepancies.

Probable Consequences of Occurrence In the event of a LOCA, the SAVS exhaust fan takes a suction in the safeguards area through charcoal and HEPA filters to ensure that radioactive materials leaking from ECCS equiprent within the pump room are filtered prior to entering the atmosphere. Since the redundant SAVS train remained operable and the joint was repaired within 7 days as required by T.S. 3.7.8.1, the health and safety of the public were not affected.

Cause of Event

The cause of this event was the failure of the rubber expansion joint retaining system on the discharge side of 1-HV-F-40A.

Immediate Corrective Action

The rubber expansion joint was reattached and properly fastened.

Scheduled Corrective Action No further action required.

' Actions Taken to Prevent Recurrence This is considered an isolated event with no further action required.
