Appendix A NOTICE OF VIOLATION Hermann Area District Hospital License No. 24-17554-01 As a result of the inspection conducted on April 29, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

10 CFR 20.201(b) requires you to make such surveys (evaluaticas) as may be necessary for you to comply with all sections of Part 20.
Contrary to this requirement, it was learned tt_ough statements of licensee representatives and a review of records you failed to make such surveys as were necessary to assure compliance with that part of 10 CFR 20.101 that limits the radiation exposure to the whole body and extremities of individuals. Specifically, you failed to evaluate the radiation drase to the whole body of an individual who prepares and injects patient doses, from August 15, 1980, to September 15, 1980. No evaluation was made of the radiation dose to the extremities of an individual who prepares and injects patient doses for the periods November 15, 1979 to December 15, 1979 and November 15, 1980 to December 15, 1980.
This is a Severity Level V violation (Supplement IV).

License Condition No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in applications dated May 9, 1977, and March 20, 1980; and letters dated June 17, 1977, and May 9, 1980.
The license application dated May 9, 1977, states that the G-M survey meter will be checked for constancy before each use by placing the G-M tube on contact with the check source or cesium-137 standard.
Contrary to these requirements, it was learned through statements of licensee representatives that the G-M survey meter was not checked for const incy before each use by placing the G-M tube on contact with the check source or cesium-137 standard since the date of license issuance.
This is a Severity Level VI violation (Supplement VII).

Appendix A Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved: (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
