The refueling outage began on May 1, 1981, and lasted throughout the month.
Plugging of first row tubes in B and C Steam Generators (S/G) was completed; the leak rate at shutdown was about 100 gallons per day.. A & D S/G had been plugged in the February '81 outage. Fuel unloading began May 18, 1981. Unioad was completed May 23, 1981, in preparation for internals baffle peening. The internals baffles were repaired by May 30, 1981. The core reload began on May 30, 1981, and continued through the end of the month.
MAJOR SAFETY RELATED MAINTENANCE:
Work completed May 17, 1981, on plugging first row tubes in B & C S/G.
Work continued on modification of walls to improve seismic integrity.
Work continued on several TMI-related design modifications.
Work continued on improvement modifications to the Plant Security and Fire Protection Systems.
Work completed on RCP seal replacement.
Work completed on baffle peening on May 30, 1981.
Work completed on mechanical snubber inspection, testing and repair as per IEB 81-01.
Work' completed on Reactor Vessel upper internals ISI.
MISCELLANEOUS MAINTENANCE:
MSIV testing completed.
Safeguards actuation test (POT 25-2 and TPT 6) completed.
Main steam safeties testing completed.
ESF actuation time response testing completed.
j Steam turbine LP cell inspection continued through month.
Containment isolation valve Local Leak Rate Testing continued through month.
LICENSU CHANGES:
I Amendment 58 - Revised to correct testing requirement of the diesel fuel transfer pumps and revised the amount of borated water required to be available in S.T.S.
Section 3.1.2.7.
Amendment 59 - Revised Containment airlock testing requirements found in S.T.S. Section 3.6.1.3.
Amendment 60 -_ Approves use of two modified fuel assemblies containing three stainless steel rods for fuel cycle 4 and one subsequent fuel cycle.
Amendment 61 - Approves the Inservice Inspection Program and grants relief where appropriate.
Amendment 62 - Revised Environmental Tech. Specs. 1) deletes all water quality re-quirements, 2) allows discontinuation of and cancels the Cooling Tower plume monitoring l

Work continued on Technical Support Center with completion of base mat and pouring of lower level walls.
Westinghouse contract employee was struck by refueling tool and slightly injured and contaminated; he returned to work the same day.
Contract employee fell 10 ft. from scaffold inside Containment and broke an arm.
Completed required irradiated fuel inspection.
