Appendix A NOTICE OF VIOLATION Augustana Hospital License No. 12-00710-02 As a result of the inspection conducted on April 29, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

License Condition No. 16 requires that licensed material be possessed and.tsed in accordance with statements, representation, and procedures contained in application dated May 4, 1978, and letters dated May 24, 1978, and March 4, 1978.

Item No. 14b of the above referenced application states the NRC Draft License Guide, dated February, 1976, Appendix C, Section 1, will be followed. Appendix C,Section I,B(4)(s) requires packages containing byproduct material to be surveyed upon re:eipt in the Nuclear Medicine Department.

Contrary t., the above requirement, it was determined through statement by licensee representatives that this condition is not being met.

Specifically, a licensee representative stated package surveys have not been performed as of the date of this inspection.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmntion.
