Appendix A NOTICE OF VIOLATION Department of the Navy License No. 04-04346-02 Naval Technical Training Center Treasure Island j
San Francisco, California 94130 As a result of the inspection conducted on May 6 and 7,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A. License Condition 15 requires that the licensee shall possess and use licensed material in accordance with statements, representations and procedures contained in applications dated May 2,1977, December 27, 1977 and April E,1979. Paragraph C.(13)(a) of NAVTECHTRACEN INSTRUCTION 5100.5E contained in the application dated May 2,1977 requires that a " monthly radioactivity survey" will be made of the radiac area.
Contrary to the above requirement, monthly surveys of the radiac area were not conducted during February, June, July, September, October and December of 1978 and November of 1980.
This is a Severity Level V Violation.

- 8. License Condition 15 requires that the licensee shall possess and use licensed material in accordance with statements, representations and procedures contained in application dated May 2, 1977.
Section 11 of the application states that radiation detection instruments will be calibrated every six months.
Contrary to the above requirement, an ANPDR/27, Serial No. A1114, which was calibrated on February 26, 1979, was used to perform a radiation survey on April 29, 1980.
This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.201, Naval Technical Training Center, Treasure Island is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective' steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
