UNACCEPTABLE WELDS DOCUMENTED AS ACCEPTABLE - HTRD-50-518/81-07 The subject deficiency was initially reported to NRC-OIE, Region II Inspector, R. W. Wright on February 6, 1981, as NCR HNPA-125. The final report was submitted on May 4, 1081. Enclosed are revisions to the corrective actions provided in our final report. These revisions were deemed necessary following a reevaluation of recurrence control actions.   • ENCLOSURE HARTSVILLE NUCLEAR PLANT UNIT A1 5 UNACCEPTABLE WELDS DOCUMENTED AS ACCEPTABLE HTRD-50-518/81-07 10CFR50.55(e) REPORT NO. 3 (REVISION TO CORRECTIVE ACTIONS IN FINAL REPORT)

We have reevaluated our action to prevent recurrence regarding welders in the report submitted to NRC-0IE, Region II, dated May 4, 1981. We have determined that the following paragraph is not adequate to prevent future recurrences:

Weldera identified in this reinspection investigation will be required to repeat their attendance of the QA orientation program and the classroom portion of the welder certification training module. Additionally, they will be individually counselled by their speciff o craft manager or his representative. Evidence of this retraining and counselling shall be documented.

- Please delete the above section and replace it with the following paragraphs:

During the above investigation, welders were identified as having presented unacceptable welds for inspection. These welders and their foremen will be counselled by the construction superintendent. The importance of quality

. . workmanship will be stressed, and the responsibilities of the

, welder and the foreman reiterated. Evidence of this -

counselling shall be documented.

Each hourly foreman will be reminded that he is assigned the overall responsibility for job-related activities of his crew.

This responsibility includes adequate supervision to ensure that welders have the proper procedures, materials, and equipment to do the job. This responsibility also encompasses the process c.* welding such that proper technique is used in

- 'fitup, preheat, welding, grinding, etc. This is accomplished by prior examination by the foreman and by ensuring that weld gauges are available at the point of usage and are used before requesting a QC inspection. If requested inspections are performed and the weld does not conform, appropriate disciplinary action will be taken.
