This refers to the routine safety inspection conducted by Dr. W. J. Adam and Mr. J. R. Mullauer of this office on April 23, 1981, of activities at Great Lakes Medical Center authorized by NRC Byproduct Material Licenses No. 12-06092-01, No. 12-06092-02 and No. 12-02301-03 and to the discussion of our findings with you, Captain Alice M. Martinson, MC, USN and Commander Gary J. Adler, LCDR, MSC, USN at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The incpection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
This inspection also included a measurement of the output of your teletherapy equipment. Our measurement was made using.a Victoreen Model 570 Condenser R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80 centimeters and the field size used was ten by ten centimeters. After applying standard correction factors for temperature, pressure, attenuation, inverse square, backscatter, timer error, and roentgen-to-rad conversion, our measurement of the output of your teletherapy equipment was 89.2 rads per minute. The most recent monthly spot-check of your teletherapy equipment, conducted on April 15, 1981, reported a measured output for the same exposure parameters of 88.4 rads per minute.
Based on the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.

Department of the Navy MAY 2 01931 During this inspection, certain of your activities conducted under NRC Byproduct Material License No. 12-06092-01 appeared to be in noncompliance with NRC requirements, as described in the enclosed Appendix A.
With respect to Items 3, 4 and 6, the inspection showed that action had been taken to correct the identified noncompliances and to prevent recurrence.
Consequently, no reply to these items of noncompliance is required and we have no further questions regarding these items.
Regarding the remaining items, a written response, submitted under oath or affirmation is required.
No items of noncompliance were identified under NRC Byproduct Material Licenses No. 12-06092-02 and No. 12-02301-03. We will gladly discuss any questions you have concerning this inspection.
