m i loss of excitation relay, preventing the EDG from starting. The blockingi 1 device was inserted durino 18 month surveillance testing as directed by i i 2 u_Lu I orocedure.

Procedure was revised to insure removal.

Visual inspection i I was conducted on other relays to verifv.that no other blocking devices i.
