Enforcement Conference On May 8,1981, an enforcement conference was held at the Veteran's Administration Medical Center, Phoenix, Arizona with the individuals listed above to discuss the findingt of a routine safety inspection conducted on March 26, 1981. During the inspection, eight items of noncompliance were identified. These items were reviewed at the enforcement conference prior to the issuance of a Notice of Violation.

This enforcement conference was announced in a letter to the licensee dated April 16,1981 (copy attached).

The meeting included all of the items listed on the agenda, but the items of noncompliance associated with the last NRC inspection at the V. A. Medical Center, Prescott, Arizona were discussed at the outset, since the Notice of Violation had not yet been issued.

Mr. R. D. Thomas and Ms. B. Riedlinger discussed the enforcement history of the hospital, outlined the items of noncompliance, and discussed the issuance of the Notice of Violation. A copy of Appendix A of the Notice of Violation, which lists the items of noncompliance is attached to this report.

Licensee representatives stated that the person receiving the whole body exposure of 1.5 rems during the period of June 24,1980 to September 29, 1980 received the bulk of his exposure during x-ray procedures.

However, it could not be stated that all of the exposure was received during x-ray procedures. The licensee also stated that i

although no formal letters were writt?.n, the individual who received I

the exposure was notified of his exposure for the period of June 24, i

1980 to September 29, 1980.

Mr. L. Camper of NRC's Materials Licensing Branch, described the j

licensing actions that were needed to get the hospital's radiation safety program back in compliance. Mr. Camoer stated that a license l

amendment request should be submitted to identify Dr. Gere as the I

Radiation Safety Officer and to add another authorized user who l

could supervise the Nuclear Medicine Departtent in his absence.

Mr. G. Spencer, Director, Division of Technical Inspection, explained the enforcement options available to NRC, and reviewed the recent changes in the NRC Enforcement Policy as described in the Federal Register on 0,ctober 7,1980.

l Mr. H. E. Book, Chief, Radiological Safety Branch, described possible future enforcement actions. Mr. Book stressed the fact that repetition of or failure to correct the items of noncompliance could result in l

the issuance of a civil penalty. Mr. Book also expressed NRC concerns regarding radiological safety and stated that an early reinspection of licensed activities will be conducted to verify corrective actions.
